![]() ![]() (3) the number of the statement in relation to that witness, (2) the initials and surname of the witness, (1) the party on whose behalf it is made, THE PRACTICE DIRECTION Headingġ7.1 The witness statement should be headed with the title of the proceedings (see paragraph 4 of Practice Direction 7A and paragraph 7 of Practice Direction 20) where the proceedings are between several parties with the same status it is sufficient to identify the parties as follows:ġ7.2 At the top right hand corner of the first page there should be clearly written: (If you haven’t read the Practice Direction yet, read it now). So litigators must know the requirements intimately. The witness statement mustcomply with the requirements in the Practice Direction. (Part 22 requires a witness statement to be verified by a statement of truth)” The rules are succinct: “Form of witness statement 32.8 A witness statement must comply with the requirements set out in Practice Direction 32. Any alteration to a witness statement must be initialled by the person making the statement or the authorised person. The witness statement must be verified by a statement of truth and signed by the witness. Each paragraph of a witness statement should be confined to a distinct period of the subject. The witness statement should follow the chronological sequence of events or matters. ![]() (vii) Give the reference to any document or documents mentioned in the margin or in bold text in the body of the statement. (vi) Have all numbers, including dates, expressed in figures. (iii) Be bound in a manner that does not hamper filing. (ii) Must be fully legible and typed on one side of the paper. The witness statement: (i) Produced on durable A4 paper with a 3.5cm margin. Any exhibit mustbe verified and identified. The statement mustindicate the whether it is made from the witness’s own knowledge and the source of any matters of information or belief. The top right hand corner must contain (i) details of the party (ii) initials and surname of the witness (iii) the number of the statement (iv) the identifying initials and number of each exhibit and (v) the date the statement is made.
